Basically, there are not many places left that are using metres and/or QFE operations. Russia has changed to QNH-operations, at least West of the Ural mountains. Where you will find operations in metres is China, both real and also here on VATSIM. Approaches are flown in metres with QNH, above transition altitude you must use the slightly different CVSM, China RVSM. To make it short: you are not allowed to just switch your altimeters from feet to metres, but you must lookup the instructed flightlevel in metres from a table and then set the corresponding flightlevel in feet in your guidance panel.
