Like: KLM745 climb to FL220 qnh 1016 According to me this is totally wrong! It should be just: KLM745 climb to FL220 without qnh. When being instructed an altitude, the QNH is usually given only for the first altitude. Thereafter it may be given again given a frequency change on the first instruction for a new altitude.
